Form Information Return

I have created a form and uploaded it to godaddy.com. When the submit buttion
is hit it seems to work, but I don't get any return information. I believe
that all the information as far as setting up the form is correct. I just
want it to e mail the information back. godaddy says it's not their problem.
The reset button works.
the for is located at
http://www.texasrealestatearbitration.com/request_arbitration.htm
If anyone knows who I can call and walk me throgh the set up and check to
see if everything is properly set up, let me know. The form is up loaded as
an .asp and not .htm. I don't know if this makes a difference or not.
 
The email; address needs to be one associated with the domain:
texasrealestatearbitration.com
The one in the form is just: realestatearbitration.com

Yes the form, if you use the FP extensions with the form, it requires the
page to be named *.htm - so it does make a difference. Change the file
extension, and you might find it works correctly.
 
If you're using FP's forms you also have to have FP extensions installed
and working...doesn't appear so this morning.
