form for collecting info

Hi all

Our company has around 200 employees who have the facility of ordering
snacks at their table. The office assitant goes to each employee and takes
orders.

Is there any facility in Outlook or in any other application (like One Note)
wherein each employees sends his request and all the requests can be
consolodiated automatically at a particular ID.

You could use a custom Outlook form for this. If you have the Enterprise
edition of OFfice 2003, InfoPath would be another solution, with better
built-in reporting capability.
