form fill button no longer on bar beside the highligher button??

A couple of weeks ago before I had to get reformatted due to BraveSentry, I
had a form fill button up there on that bar right beside where the
highlighter is at.

I have the highlighter and several other buttons now, but I can't seem to
find the form fill button anymore.

Anyone know what happened to it?

And I have all the boxes checked, but my autocomplete does not fill in forms.
