i am wondering how to adjust the width of form fields?
I have two problems:
#1 all of my info doesn't fit into the form
#2 on the check boxes the info telling what the box is for isn't next to the
box

Before you get too concerned about the layout of the form, you need to be
aware since it is *not* secure, and you are asking people for credit card
information, you are opening up the door for some serious liability issues.

1. You would need to shorten your text to fit, but see #2 below which will allow you to make the
field a bit longer.
2. The quantity can be made smaller since you only need it wide enough for 2 characters,
3. When you insert radio button, press the spacebar once or twice then type in your text.
