Form Field Result Problems

Hi,
Can you help me determine why I am getting strange results from this
form:http://www.jiffydelivery.com/_vti_bin/shtml.exe/jiffydeliveryservicequote.htm.
Specifically:
- The first City, state, and phone fields pick up information from other
fields or the results are repeated multiple times (see example below).
- The check-boxes values (yes) are repeated.
- The last City and State fields do not even show up.

This is currently run a Linux server but I tested on bCentral server and got
the same issues. Could the Google toolbar have anything to do with this?

2. You must be publish via FP's http mode to a server that has the FP extensions via File Menu |
Publish Site and entering the URL as http://www.jiffydelivery.com

Thomas, I am publishing to the server (w Frontpage extensions turned on). I
have the normal options: File/Publish Site/FrontPage Server checked on remote
server options. I have been publishing changed pages.
 
Make sure you do not have Optimize HTML enabled/checked.

Republished with "optimize HTL" not checked and have the same issue. The
City and State fields are picking up information from special instructions
and two city and state fields are not even showing names/results. I have
deleted the labels and fields several times to try to start clean.

You need to rename all fieldname without spaces, periods or numbers as the first character or any
other special characters. All fieldnames must be unique on the form, currently you have 2 City, 2
State, etc.

Make sure you also rename the fields.
