Form Design for a linking or union table

I have a many-to-many table relationship. I am creating a linking or union
table so that the end user can pair up each side of the many-to-many.

So how does one design a form to make it easy for the end user to establish
these links? Is there a recommended design or method?
 
One approach is to use combo boxes to select "one from table A, one from
Table B" to associate the two.
