You design completely separate web forms. Docs start at
http://msdn.microsoft.com/exchange/ with discussions in the
microsoft.public.exchange.applications newsgroup. The learning curve is
somewhat steep, since none of your Outlook programming skills are
applicable.

How do you go about publishing custom forms for use in Outlook 2000 Web
Access (OWA) if your using Exchange 2000? How can you share user-defined
fields (created in custom forms in Outlook 2000) with people out of the
office using OWA? If possible how easy is it to setup and maintain?

Visual Studio or whatever you prefer to use to write ASP pages. Tom Rizzo's
book is a start, but you'll probably learn just as much from the MSDN docs.

What program(s) or tool(s) do you use to create these custom forms? Can
you recommend and good books on the subject?
