Form Buttons Not Working

I've created a questionnaire in Word, saved it as a template, and then opened a document based on it to test its usability. When I select an answer, move to the next question, and answer that one, the previous response disappears. I've followed the instructions to the letter and have re-tried several times, but nothing works.

Controls Toolbox or something else? Is it protected as a form? Which
version of Word.

that we can get an idea about what's going on.

I created the document in Word 2000, using ActiveX controls from the Controls toolbox. I have since re-created the document with the more basic Form fields and had no issues with that; I just prefer the visual impact of the ActiveX controls. And yes, I did protect the document before testing it

I really appreciate your help with this!

OK... I can't recall having ever seen behavior such as you describe with
the ActiveX controls, so we'll need to do some more digging.

1. What's the macro security setting on your machine
(Tools/Macro/Security)?
2. If this template/document is opened on another machine does it exhibit
the same behavior?
3. If you start Word with the /a switch (START/Run; type the following,
then press ENTER: winword.exe /a ) do you still see the behavior?
