Form asking for password

Can someone please take a look at this for me?

This used to work perfectly and I have no clue what happened and don't know
when. I switch from CPanel to V-deck with my hosting company (Ipowerweb) so
could that be the problem?

What happens is when you hit submit it asks for a username & password....I
just want it emailed to me...ugh!!!! When I right click on form properties
and enter my email addy it says
'this form cannot be configured to send results via e-mail. The website
might be located on a disk based location" blah blah blah....

In the form properties it has been specified that the results are saved to
http://scentsofnewenglandcandles.com/Private

This should be _private/formrslts.csv - in particular note the lack of
http://scentsofnewenglandcandles.com and the _private folder, the actual
filename is not so important.

Change this in form properties. The .csv implies saving the results as a
comma separated values file - you may wish to use some other format, in
which case the name and extension may be changed.
 
Thank you Ron...but I guess I'm not grasping what I'm supposed to
do....should I not have a folder called "private"? should I insert an excel
document into this "private" folder? Sorry I am just drawing a blank
here...I really do appreciate the help...
 
I notice that the results location has changed to
http://scentsofnewenglandcandles.com/_private/formrslts.csv

You should already have a folder named _private (starts with an underline,
all lower case. This folder is created by the FrontPage extensions.).

Open the web in FrontPage, then open the form page. Right click anywhere
inside the form, and choose Form Properties.

Change the sendto filename to
_private/formrslts.csv
removing the http://scentsofnewenglandcandles.com/

Click Options button, and change Format to "Text database using comma as a
separator"

This format can be loaded in Excel or Access - there is no need to add an
excel file to the website.
 
You need to have/use the _private folder not the Private folder that you are currently trying to
use. FP will create the .csv file.

Yeah I accidently deleted the "private" folder on the server side. I have
asked my hosting co if uninstalling the FP extensions and then reinstalling
would some how put the folder back in there without compromising the site but
haven't heard as of yet.
