footer with form field type info inserted

A document that is a form with fields to be completed. At the bottom of each
page a form field is required repetitively. Cannot put form fields in footers
so how can this be provided.

Put in one form field on the first page (not in the footer), and put a
cross-reference to that field in the footer to repeat its contents on
each page.
