Your diskette can have defects, the floppy drive can be defect, the
floppy drive cable can be loose or connected wrong or the drive could be
not configured or wrong configured in Bios or Windows.

Check the configuration and hardware.
Try the floppy, drive and cable on another computer.

Besides all this, many users have a problem with the floppy drive in
Windows XP, the cause of this is not known yet.
 
One more thing to try if all else fails:

Take the A drive out of the casing. If it works that way it means there was
interference from the hard disk above or below it. Screw back the A drive to
the casing and move the hard disk a little. By trial and error you should be
able to reduce the interference from the hard disk drive and allow your A
drive to work. The cause could be that Xp is very sensitive and particular
about reading data from the A drive reading head and any slight corruption
will force it to report an error.
