fixing a broken page

I accidentally deleted my contact info page. Could someone please go to
www.pbzoneardmore.com and look at contact us and tell me how to redo this
page? (I mean literally tell me how) I cannot get my web designer to call me
back and I don't know how to do this on my own. Thanks

Your contact page is there, but the form (in an Iframe) is missing
Create a new form using File new page (use the Forms Wizard from the page templates) and name it con.htm
