How do you set up Windows Explorer so that file
extensions are always shown?

Folder Options > View tab > in Advanced Settings, uncheck the box marked "hide extensions for known file types" > apply > OK

I'm using XP Pro so it may be a bit different if you're
using another system:

Open up Windows Explorer.
Click on the "Tools" pull down and select "Folder Options"
Click the "View" tab
At the bottom of the window, you should see boxes for
Advanced Settings.
Scroll down until you see "Hide extensions for known file
types" next to a box. Remove the checkmark if there is
one.
