Fields in custom appt form aren't in calendar folder. How??

I've created a custom page on the Appointment form and made periodic
revisions. How do I get ALL of the "User Defined Fields" to reside in the
Calendar folder? I sent the form to a workmate, and he can see it and use
it. But, when we try to make a new "calendar view", most of the fields are
not available and we can't find them. Where are they? How do we make them
reside in his Calendar folder? Thank you kindly!
