I have a site which has been working fine for years and now all of a sudden
when a customer uses the contact form they get a Login form asking for
username and password pop up when they click on submit.

What could cause this and why after 2 years does it suddenly start
happening.

something happened server side to alter the permissions on the file you're
saving form results to.
try this;
open your site live, find the form in question, right click and chose "form
properties", browse to the file you want to keep the results in and OK out
of the dialogs, save the page, click tools/ recalculate hyperlinks, close
the web, open the form in a browser and try to submit.

Thanks Chris I tried that and still the same.

I also went onto the server to check the folder permissions of the file with
results and it has read/write/modify permissions as it should have.

This is a real puzzler.

have your host re-set the FrontPage extensions.
this is obviously a permission error.
it's this file: _vti_bin/shtml.dll/cu.htm
