feedback form field names

i have created a feedback form on our website (found at
www.ltfexperts.com/contactus2.html).

when the user clicks the submit button, the field names don't appear. in
place of Name, for example, it says Username6, and so on. any suggestions
on how to make the actual field names appear?
 
You need to select each field, right click and select field properties, and then name each field.
Field names can not have spaces or start with a number.

I selected a field, right clicked, but "field properties" was not a
selection. The Property choices were as follows: Form; List; Cell; Table;
Page...
 
With web open in FP, and then the page, select a form field, then right click and select "Form Field
Properties" or use ALT+Enter
