Federal Poverty Line Table

I work in the healthcare industry and in order to qualify a patient for a
discounted fee I need to be able to calculate where they fall in the Federal
poverty line (FPL). I have to tables: 1 for patient data. and 2. federal
poverty line values

Two fields from the patient table are needed to determine the federal
poverty level: 1. # dependants 2. annual family income

There are four levels of FPL: 0-50%, 51-100%, 101-150% and 151-200%

The FPL table is as follows:

50% Federal Poverty Line 100% Federal Poverty Line 150% Federal Poverty
Line 200% Federal Poverty Line

I need a query or something that can be put into a report classifying the
patient into the various FPL levels.

Please help me...
 
You need to reformat your FPL table to look like this --
