faxing via calling card

I am having difficulty configuring the calling card feature on my fax modem.
Does anyone have knowledge of a better "How To" than is provided in
Hewlitt-Packard Help and Support?

What card? What is the sequence of events when making a call? Time for
pauses, etc. Someone can give you the exact steps with that info.

I use an MCI card from Costco with no problem except that I have never been
able to use the address book correctly. I must enter the name and phone
number manually.
 
Be advised that KB article 239888 applies to Windows 2000 ONLY, it does NOT
work with Windows XP:
