explorer.exe crashes - desktop blank

Win 2000 server with SP4.

Intermittently, explorer.exe seems to crash and the desktop goes blank.
Can be restarted by running explorer.exe again.

Heres a snippet from drwatson.log :-

Application exception occurred:
App: explorer.exe (pid=284)
When: 14/09/2006 @ 11:58:00.109
Exception number: c0000005 (access violation)

BTW. Have already tried the fix which involves renaming shdoccvw.dll.
Doesnt make any difference...

The thing is this problem is occuring on six servers that have all
recently been built and had SP4 and additional patches applied. Also,
installed is Java, and a bespoke application.

Have looked at the some of the microsoft articles you refer to and,
also, the troublshooting link you gave me.

None of it seems that relevant - I dont get any error messages at all -
explorer just dies....
