Hi John,
Try deleting your profile-specific Excel.xlb file. This will be located
in the folder:
C:\Documents and Settings\<username>\Application Data\Microsoft\Excel
This isn't guaranteed to solve the problem, but it's worth a try. Excel
uses this file to store information about its toolbar configuration, but it
frequently becomes corrupted and cause various problems like this. Excel
will create clean new default version after you delete the old one, but you
will lose any toolbar customizations you've made in Excel.
