Excel Functions-Forms

I'm trying to create a form in Excel which asks the user a
series of questions and the answers are scale based (i.e.
on a rating level of 1 - 5). There are approximately 40
questions and in it's current status if the value for
question one is selected when answering question 2 and on
the previous selection is automatically cleared. What
have I done incorrectly and how do I fix it?

Sorry, I may not have explained it all too well. I'm
using Radio Buttons for the rating scale. Here's an
example of the questions within the spreadsheet...e.g. if
I select 4 which equals "Satisfied" in question one; when
I answer the next question, my previous selection clears.

I would think that that is because all of your radio buttons are in the same
group, rather than separate groups per question. Only one radio button per
group can be active at any one time.
