Not on any website that I know about. I've created one for my bill form
which uses an envelope template rather than the envelope wizard. I put it on
a custom toolbar button that displays in the menubar when the form template
is being used. It creates a separate document. I'm sure it is possible to
mimic the wizard, but I like having a separate document.
If you want to look at it, send me an email at:

Replace the " (at) " with "@" and the " (dot) " with "." and delete "remove
this" to get a real email address.
Note, you do not want to use your real email address in the header of
