enable "File Download" Prompt for ALL files?

Just upgraded from IE5.5 to IE6.0 and the Security settings seem to have
changed for the worse.
Before, I was prompted before ANY Internet downloads would occur. Now, I'm
only prompted for .EXE files!

In IE6.0 tools-> Internet Options-> Security-> Internet
Security Setting = Medium

under Custom: Downloads: there's no prompt button as in IE5.5!

there's only disable / enable. Disable is no good to me because it totally
blocks all downloads.

also checked under the Advanced tab for some kind of file prompt. No luck.
Is there some way to be prompted before ALL downloads for my protection?

How could removing this prompt option improve the security features of IE?

Go to Control Panel > Folder Options > File Types. Highlight the file type
you want to change. Check Advanced button. Check "Confirm open after
download"
 
I'm using Win98.
so in Explorer > View > Folder Options > File Types
there are about 200 types. The default setting for most of them is "confirm
open after download"
and it does NOT work.

e.g. download historical price data in a .CSV file in Yahoo Finance, a
possible ?confirm window? flashes up and then dissappears. Then the file
goes ahead and downloads anyway w/o asking if I want to save it to disc!

e.g. try to download an Adobe PDF file to disc. There is NO confirm window!

I know these are essentally harmless file types BUT how many other file
types that I don't recognize are downloaded w/o my consent!
