Emailable forms

I have created a form in Outlook XP, it is very simple;
one text box and three check boxes. I have saved it, ran
the sform then send it. I send the test message to myself
and it works, but when I reply I lose the second tab which
contains the form. What's the problem? How do I get the
recipient to sucessfully reply through the form (second
tab)? Thanks! To all the americans, have a very happy
thanksgiving; to those from the rest of the globe, enjoy
the rest of your weeks'
 
The form used for the reply is designated in the properties for the Reply action on the (Actions) tab in design mode. Note that this will work only if the recipient has access to the published form definition, as they might in an Exchange Server organization.

FYI, there is a separate newsgroup for Outlook forms issues -- microsoft.public.outlook.program_forms.
