email template

My fill-in form was created as a template. When I copy it to email, only the
clipboard icon shows up---not the form. How to display the form when pasted
onto email??

Could you explain a bit? What's a 'fill in form'? Are you using
Microsoft Access, or some other software?

I created it in word---saved as a template. It's a form w/check boxes and a
fill-in text book at the bottom.

Then you might want to ask the question in a Word newsgroup. This
newsgroup is for a different program, Microsoft Access.
