I need to send an email to clients that is a questionnaire and would like to
do it in a form with a respond button so that it as easy as possible for the
client. I can not figure out how to make this happen. I do not have a website
that the response would go to. Is this possible?
 
Create a fillable PDF form and email that as an attachment, have them fill
it in and email back to you.

Thank you for the reply. Unfortunately that does not do what I have in mind.
I want to be able to email a form with a variety of check boxes, have the
client pick the appropriate answers and click a button to email it back. The
easier I can make this the better the response I will receive. Thank you
 
If you mean you want a standard HTML form in the body of an email, this may
not be possible. It would be better to set up a web site with the form on a
page because HTML forms require a web server to function properly and may
not do so correctly from an HTML email.

Also,the email programs your users have (don't assume they all use
Outlook/Outlook Express) may or may not support HTML (and so won't display
the forms) and without a web server the form isn't likely to send you the
results of the survey anyway as above.
