Edit 'Check Full Name' form in Contacts

Thanks to "Sue" for your help in getting me going in
programming outlook!

I now want to edit the 'Check Full Name' form that you get
when you click the 'Full Name...' CmdBtn . I would like
to delete middle name and add to the titles in the cb.

Appreciate it!
