I have followed instructions and set up my display to 1. primary and 2. LCD.
Both are showing up in my display propeties. Originally, I had a blur on the
wall until I adjusted the resolution. However, only the desk top background
is showing - no icons and no presentation. I've played with the hardware
acceleration and reviewed many posts - but have not come across this isuue. I
would like to eventually get to presentation mode in set up show. Can anyone
help?

This is correct! In dual monitors you don't see the icons - that way you can
have your background black (or whatever you like) so that it doesn't distract
when you are not in slide show mode. What happens when you start a
presentation? Go to Slide Show | Set up Show and check to show Presenter
View. You may have to adjust the Multiple Monitor settings in the box above.
Your slide show should be on the Primary Monitor. Hope this helps. Post back
on how you get on.

Just to help you understand what youre probably seeing

Image that your desktop extends into a space to the right (maybe left) of
your monitor. That space is showing on your 2nd monitor. Make sense. You have
no icons in this space hence blank desktop but you will probably find that
you can drag things right off the side of your monitor into this "space" to
show on the 2nd monitor.

Hope that makes some sense

Actually, it does. I went back to display properties-settings and read at
the top that I can drag the icons of the monitors to match the actual
physical arrangement. I haven't tried that yet.

I did launch my PP while connected to the LCD (still showing the desktop
w/no icons). I went to Slide Show - Set up show - and checked Show presenter
view - OK. I still only had the empty desk top. I checked to make sure that
my desk top properties were good - 1. computer 2. LCD desktop extended. It
did not work. I went to ATI to see if I had the latest drivers (ATI Mobility
9000). Check the ATI settings - display settings advanced and both PC and
monitor were showing. Could it be something with the resolution?

Is there a proper order? I am sure I am missing something really obvious.
though I would really like this too work. My goal is to have a view of all
the slides on my PC and click on only those that I want the audience to see
as I speak. This is SO frustrating.

I figured it out!!! This is what I did (I had already gone through the steps
of extending my monitor):
1. Went to display properties - settings and dragged the icon for the #2
monitor (LCD) monitor to the physical position. I was then able to see my
cursor on the LCD when I moved it off the PC desktop.
2. Opened another PP presentation. Went to slide show - set up show and
noticed that under "multiple monitors" it said display slide show on primary
monitor in the drop down box. This was grayed out in my original
presentation. I changed it to display show on extended monitor. I also
noticed that the show type was "presented by speaker". Clicked on ok - and
it showed up on my LCD !!
3. closed out and went back to my original PP that was giving me trouble.
Went to set up show. Now the display slide was no longer grayed out and was
set to show on extended monitor. However, the display show was set to
"individual" . I changed it to "presenter" and clicked OK - and it WORKED!!!
4. I am very happy about knowing how to do this. Though, I am sad that I
cannot change the view to all slides. I have the frame on the side bar.
Maybe in the next version of PP this will be an option.
