DSL with name and password connexion not available

I want to install my speedtouch usb ADL modem but when i
arrived to
"Connect using a broadband connection that requires a
user name and password or Connect using a broadband
connection that is always on." i cannot enter my password
and my name because this option is not available and in
this windows only the last option is available "connexion
dsl without name and password" so i could not be
connected to my FAI.
