This is how my table looks like:

School Task Start_Date Finish_Date
Act_Date
Main El email staff list 3/01/05 3/05/05
Main El send survey 3/01/05 3/05/05
Main El return survey 3/10/05 3/15/05
Blue El email staff list 3/01/05 3/05/05
Blue El send survey 3/01/05 3/05/05
Blue El return survey 3/10/05 3/15/05

My problem lies when I try to create a form...The "Act_Date" is the only
field that the user will be updating. However, how do I show this on a form
if my task field has 3 different entries (e.g.: email staff list, send
survey, return survey).

By basing the Form on a query which displays the desired records.

Since I have no idea how you decide what those three entries might be,
nor the meaning of the date fields, nor just what you're trying to
accomplish, it's a bit difficult to suggest how you would construct
this query.
