Developing Patch Updates

How do I go about building & deploying patch updates?

Currently I have a web application - which clients
install. Every so often I need to deploy an update.
Clients have to uninstall the current version - before
installing the new.

Is it possible to simply deploy a patch? and what
considerations do I need to take into account?
