The Desktop is redrawn. The position of the icons isn't moved.

Refresh means to update displayed information with current data.

You can also hit your F5 key to refresh the Desktop. Does the same thing as
right clicking the Desktop and clicking on Refresh.

This registry key stores the Desktop layout, it also gets refreshed when
performing a Desktop Refresh. That is, your Desktop layout gets updated and
remembered.

HKEY_CURRENT_USER\Software\Microsoft\Windows\Shell\Bags\1\Desktop

Windows automatically refreshes this information periodically. How often, I
have no idea. But if you have ever had explorer.exe crash or disappear and
then reappear to find the Desktop layout not saved exactly as you had it
before the crash, you might think that it's not often enough. ;-)

Event Source: Winlogon
Event ID: 1002
Description: The shell stopped unexpectedly and Explorer.exe was restarted.
