Have you guys found a fix yet? THis is very annoying. Im
using Icon Packager and prior to format, worked fine

I've noticed the same thing.
I'm running Windows XP Home on a Athlon XP system, with a
nVidia Geforce 4Ti 4400 video card, and running nVidia's
WHQL drivers 52.16.
I'm using a number of Plus! desktop themes that I have
modified slightly (edited the .theme file) so that they
will work under XP with the "Luna" system theme (silver).
By the by, did MS know how confusing things were going to
become because of using "theme" as a generic term for both
the old Plus! desktop theme packages, and for the newer XP
system style theme?
Any way, I was using this method for the longest time with
out ever seeing a problem with the icons. Around a month
ago I did a reformat/re-install of Windows XP. It had been
running like a champ for well over a year, but I felt it
was due for a bit of cleaning out of the "cob-webs". I used
to have to do 95 every 6 months and 98SE every 9-12 months,
so it's not a big deal for me to do this.
Well now Windows XP seems to forget the custom icons
included with the Plus! theme package, and goes back to the
default "Luna" ones. There doesn't seem to be any rhyme or
reason for this. I've seen it happen for the following reasons:
Right clicking and hitting refresh for the desktop
After returning from a fully open window (say IE or Outlook Express)
After returning from a full screen game
On a soft reboot
On booting the computer after it's been off.
These seem to be the circumstances behind the problem, but
it doesn't always happen due to these circumstances, more
at totally random times. Something else I've noticed, is
that the "Recycle Bin" icon seems to be having a problem
too. Some times it won't change from the empty to full icon
(or the reverse depending on what was displayed before the
problem occurred).
I do use the tweakUI for XP, but all I've done is remove
the arrow. As well I've tried the rebuild icon tool and on
doing this the default icons are used. I've far as I can
see it's either a Windows XP problem or a nVidia driver
problem.
