Database of Contract Address

I would like to create a contact address database for my boss with all kind of
facilities. I am working with a bank as Executive Secretary of my boss. We
have
lot of contact telephone for many branch managers, internal contact numbers
in our head office and branches in all part of the country. Please suggest
one.
Fields I need : Name, extention#, tel#, Mobile#, location, Dept (if there is).
 
well, for a working database that's already built, you might take a look at
Microsoft's website, the Templates section, and see what's on offer there.

and have you considered using MS Outlook to store and organize your
contacts?
