Data disappears when custom form is sent

I have a query regarding a custom form I've published to the organizational
form directory of outlook 2003.

What I'm trying to achieve is for users to complete this form to send an IT
request to me for action. The form I've created is accessible from each
user's workstation but when they fill it out and send it to me I get the form
in a template-style format (ie. no text, screenshots etc, just the form)

Any help greatly appreciated. Thanks
 
Did you design the form's read layout so that it contains controls bound to
the custom fields that hold the data? When you published the form, did you
leave the "send form definition with item" box unchecked?
