Customized Form not loading

Hi

I have a customized contact form and it's under public folder. I have this folder sharing with other employees in the company and customized contact form is published to that folder. We had people entering information to the folder but half way through some of the employees is getting a message saying the form would not load and to contact the network administrator. Can any body help me with this problem? I have cleared cashe on my pc and don't have a problem loading the customized form on my pc. Does this mean that every pc that is giving this message needs to be clear cashe. Please advise. Thanking you in advance

Yes, any machine receiving this error would need to clear the local forms
cache.

Sure would have been nice if you'd mentioned your Outlook version and build
number.
