Customized form for a printer created but not visible

I created a customized form under the Print Server
properties on a Win2K SP3 machine.
I have two kinds of printer shared on that server
- one connected directly to the net over a JetDirect
- one connected to the Server LPT1 port
Both are using the general HP HPGL/2 plotter driver, but
both doesn't show in the media formats, the customized one
I added in the Print Server properties.
Any idea ?
