customising contact form

In Outlook 2003, is it possible to customise the contact form?

Firstly, on the General tab there are four fields in the Phone numbers
section with drop-down boxes that allow you to select which phone numbers
are displayed for that contact. Suppose I choose Home phone, Home fax,
Mobile and Pager. Can this particular selection be set for all existing
contacts and/or as the default for new contacts?

Secondly, can another field be added on the General tab, such as a fifth
phone number or the department (from the Details tab)?
