customer support survey

my boss wants me to create a suvey based database to send out to all of our
facilities to rate our department based on our service to them. i have the
questions that need to be used in the survey but i am not even sure where to
begin. i need it to be email based if possible and it needs to be able to be
submitted via email. then from their i need to be able to track and report
the feedback. i have a general idea of how to do it, but i am new to access.
so please access godz help a worthy subject out

If you really want to automate all of this with emails etc, you should
consider a solution like http://info.zoomerang.com/. We use it at work for
internal surveys of employees.
