Custom paper size not available in Access XP?

Despite adding a custom size to the printer server
properties, there is not a choice to use it in the Access
page setup for my 4" x 6" index card which was easy to use
in Access 97. Can this be changed?

The paper size is set in the Printer properties dialog, not in Access.

Start + Control Panel + Printers + Properties + Printing preferences

Click on the Paper Size drop-down.
Find the 'User Defined Paper Size' (at the bottom of the list).
Click on it and a dialog box will open for you to set the size.
 
I know this. Unfortunately there is no user-defined
listing despite trying 3 different drivers for the HP LJ6P
printer. I have tried adding a size to the list in
Printer Server Properties to no avail. While I see this as
a printer issue as well, oddly, MSWord listing of paper
sizes included a "custom" size.

automatic select
auto
tractor feed
manual paper feed
