Custom Forms

I created a custom interlibary loan reply form.
I then created the message form that begins the interlibrary loan process.
Under actions I set reply to connect to the ill reply form.
I published both forms to a folder on the exchange server.
I send a form from my email to another user and they can view the initial
form, but when they attempt to reply it says that the form cannot be
displayed.
I feel like the problem has to be with the action set to reply that it is
trying to pull the ill reply form from my computer instead of from the public
folder on the exchange server, however I cannot find a way around it.
What am I missing
 
Forms published to a folder can't be used as reply forms. The reply form must be published to the Organizational Forms library on the server or to the user's Personal Forms library.

Thanks Sue, Now can you tell me how I can remove the forms I did publish to
the Public Folder?
 
On the folder's Properties dialog.
