Custom forms with check boxes

I have created a custom form in outlook 2003 which includes several check
boxes. The problem is that when the completed form is mailed back the checked
check boxes often come back blank. The rest of the data from user defined
fields arrives fine. I havve searched but can find no reference to a problem
like this. Any ideas??

The most likely cause is that the check box is not bound to an Outlook
property on either the compose layout or the read layout.
 
Thanks for the quick response Sue. I'm a bit confused when you say "is not
bound to an Outlook
property" The check boxes are user defined and placed on both layouts.
Checkbox13 for example is in the same location on both layouts. Is there
another property I should be looking into? The only properties under the
advanced tab that do not have values are Control Source, Group Name and Tag.
Could you clarify? Thanks
 
Bring up the Properties dialog for the control and switch to the Value tab.
At the top of that tab, is a field selected? If not, then you need to choose
or create one so that Outlook has a place to store the data that you are
collecting with that check box.
