Custom forms storage/back-up

I have designed and published my own form for Outlook Contacts which i
am now using, but it took me a month of Sundays to work it out, so i
don't want to lose it!

Is it possible to back-up custom forms in the event of re-installation
being required due to system crash, hardware failure etc?

Version is Office 2003
 
File, Save as Outlook Template (oft) .... or make a copy of the pst they are
published to (and delete all messages etc to reduce the size)
