Custom Forms - Outlook 2000

I have been using a custom form for data entry in form view and list view
for referencing the items in my contact folders.

Due to corruption of one of 30 contact folders, I was advised to create a
new Personal Folders file. I have been able to do that and my 30 contact
folders are there but when I try to create a new contact, the standard
contact form comes up instead of my custom form.

Properties tells me that the custom form is used for new postings and the
list view shows the custom fields as before. The form selection box in
Properties does not show my custom form in the Personal Forms Library.

How do I find the custom form and get it into the Personal Forms Library for
selection?

You need to publish and assign the form to the folders.

Why are you using 30 contact folders? Use one (or two) and categories.
 
Where do I find the Custom Format form to publish? It must still be in the
old .pst file but how do I get it from there to the new file. The list view
contains most of the fields in the custom form, so it must be somewhere in
my new file also.

I am sure glad I had my contacts in 30 sub-folders. When I got the
corruption, it only effected one folder and about 22 contacts out of my
total of 3500. I do use categories on several of the folders but some
folders are essentially a category initself.

All of a sudden, my custom form appeared with the existing and new contacts
but it still doesn't appear in the "Choose forms box" so I am afraid it will
again disappear.

What do you think might be happening?

Create a new profile with the old .pst file as the main data store, no email
accounts. You should be able to access the Personal Forms library with the
Forms Manager and save any form as an .fdm file, which you can then use to
install in your other profile.
