Custom Form

I am using Outlook 2000 on Windows XP. I have created a
form and am using User Defined Fields in my form. The
form is NOT set up to use Word as default editor.
I add the fields to the workspace, publish the form to
the Personal Forms Library (thinking this will
incorporate all changes). Then I run the form and e-mail
it to myself.
The information looks fine on the screen, but, when I
print the e-mail the information does not show well.
1. It prints the fields in alphabetical order instead of
how they appear on the screen. I tried to overcome by
numbering all fields, but, really do not want to do this.
2. My Number values are set to 2 decimal. If both numbers
to the right of the decimal are .00, then these zeroes do
not show on the printed e-mail.
3. And the most aggraviting of all, fields show on the
printed e-mail which are not currently on the form or on
the screen. These are fields which I originally put on
the form but deleted from the form AND from the Field
Chooser.
Can anyone help with the "ghost" fields and how to make
the receipients e-mail print better?
