Custom Form Isn't

I created a custom form using "message" as the starting
point. The form looked great on screen but when I tried
to send or print it looked like a standard blank message
form.

Anybody have any idea what I'm doing wrong? I'm using
Outlook 2002.
 
You probably didn't create a custom read layout, only a custom compose layout. Message forms, by default, need both.

Also, see http://www.slipstick.com/dev/customprint.htm for various solutions to the limitation that Outlook forms don't support WYSIWYG printing.
