Creating Surveys in Microsoft Word

I would like to know can you create a survey in Microsoft word. Do you have
to use templates to create surveys or is there another way?

Are you talking about something that is "dynamic" and changes based on
selections? if so, it will require VBA to perform. If you just want
checkboxes, etc., check out the following link (paying particular attention
to the Dian Chapman tutorials). This option shows you how to use Table
cells to hold various data that can be filled out electronically (or printed
out and handwritten).
