Creating new Word forms

I want to create forms in Word that can be e-mailed as
attachments (example: New User Request - add a new logon &
e-mail account). A manager would open it in Word, fill it
out, send it to IT as an e-mail attachment. We'd do the
work & send it back as a confirmation.

Where can I go to learn about creating forms? The help
system in Word doesn't have much to say.

Word's help does have a lot on this, but you have to look under "online
forms." That said, the Dian Chapman articles are far more complete. (IMO)
