Creating Forms

Hello I am a Principal of a small sized school and would like to cut down on
the amount of paper used. I am trying to design a form so that a teacher can
write a behavior (discipline referral) form and send it to me for action and
save a copy for themselves. It must have a way for me to reply back to them.
I have been trying to do it, but need a little help getting started.
I would need an box to send to me, maybe a cc so they can send to themselves
or file. Place for a student name, drop down box with a list of infractions
and a place to write a short narrative.
Maybe outlook isn't the program for this. So someone can set me straight.
