Michelle, first things first: Create your tables. To create a form based on
a specific table, the easiest way is to select the table and click the New
Object: AutoForm button on the toolbar. That gives you a "quickie" form.
Next easiest way: Go to the Forms page in the database window and select
"Create form using wizard." Follow the wizard steps, and you'll get your
form.

Once you have a properly normalized (and if you don't recognize the
term... go to http://www.mvps.org/access or http://support.microsoft.com and search for tutorials!) set of tables,
you can open your database, select the Forms tab, and either use the
"Forms Wizard" to create a new form using one of Access' canned
designs; or simply create a new Form and manually put textboxes, combo
boxes, and other data interaction tools onto it.
